Community Health Needs Assessment

As Geisinger Health System prepares for our next Community Health Needs Assessment we are issuing a request for proposals (RFP) for assistance in conducting this health needs assessment for Geisinger's fourteen (14) county service area, including: Centre, Columbia, Juniata, Lackawanna, Luzerne, Lycoming, Mifflin, Montour, Northumberland, Schuylkill, Snyder, Sullivan, Union, and Wayne counties in central and northeastern Pennsylvania.

Key functions of this Community Needs Assessment include:

A. Obtaining statistically valid information on the health status and socioeconomic/environmental factors related to the health of residents in the 14 listed counties.
B. Supplementation of the general population survey data currently available to us as an organization.
C. Ensuring community members, including those within a broad based racial/ethnic/cultural and linguistic minority group, are the primary participants in the needs assessment survey process. In addition, educators, health-related professionals, media representatives, local government, human service organizations, institutes of higher learning, religious institutions and the private sector will be engaged at some level of the survey process.
D. Development of accurate comparisons to the state and national baseline of health measures utilizing the most current validated data (i.e. 2013 Pennsylvania State Health Assessment)
E. Utilization of data obtained from the assessment to address the identified health needs of our service area
F. Obtaining project reports for all licensed facilities within this agreement.
