The Social Services department at Geisinger-Shamokin Area Community Hospital coordinates the discharge planning process for our patients. The department assists in getting patients home as safely as possible, or in placing patients in other facilities for further care.
Some of our discharge plans include sending patients home, with or without home health or medical equipment, assisted living placements and nursing home placements. Social Services also assists with discharges from the emergency room, which are usually emergency placements.
We assist in providing living will information, medical assistance applications or any other help and guidance that a patient may need.
Generally, whenever a patient informs a nurse of any type of social problem, nursing contacts the social services department immediately to address any given situation. This service is provided for all patients seen at G-SACH.
