Health system creates Geisinger Home Health & Hospice

DANVILLE, Pa. – Geisinger announced a consolidation of its home health and hospice services as one entity, Geisinger Home Health & Hospice. Those facilities now coming under this new umbrella include Columbia Montour Hospice, Geisinger Home Care, SUN Home Health and Hospice, and Holy Spirit Home Health.

“We see this unification as an opportunity to continue our mission to provide patient-centered, high-quality home and hospice care while further focusing efforts on collaboration,” said Janet Tomcavage, chief population health officer for Geisinger. “We are committed to providing a focused approach to caring for our most vulnerable patients in these services across the Geisinger footprint.”

Geisinger Home Health & Hospice services include nursing care, physical therapy, speech therapy, occupational therapy, social work, pain and symptom management, emotional and psychosocial support, community resource planning, and grief and bereavement support.

Geisinger Home Health & Hospice will remain at current service locations in Danville, Bloomsburg, Wilkes-Barre, Northumberland, Montoursville, Lewistown and Camp Hill. Current staff, phone numbers and referral processes will remain the same for specific locations to ensure there are no gaps in care coverage.
