How to manage your child or loved one’s health with MyGeisinger
Managing both your and your loved one’s health can be confusing, but not with MyGeisinger. From messaging your care team to viewing your loved one’s immunization history, you can access everything in one place.
If you are a caregiver of a child or an adult loved one, you can use MyGeisinger to help manage their health. With MyGeisinger, you can access information about their care at your convenience, communicate with their care team, manage prescriptions, schedule appointments and more.
Depending on the age of your child, parent or loved one, there are simple steps you must take in order to access their health records.
How do I get access to my child’s health records?
To request access to your child or children’s medical records, you must:
- Have access to your own MyGeisinger account.
- Be the birth parent or legal guardian of a child under 18 years old.
- Complete and return the appropriate parental authorization form.
If you don’t have an account with us, one will be created for you so that you can access your child’s records through MyGeisinger. Depending on the age of your child, you will have to fill out a form to obtain their medical records.
- For children 0-13, complete the under 14 proxy form.
- For children 14-1, complete the 14-17 proxy form.
- For children 18 or older, complete the caregiver form.
In order to not lose access to your child’s records as they turn 14 or 18, be sure to submit the appropriate form prior to their birthday.
How do I get access to my adult loved one’s health records?
To request access to your adult loved one’s medical records, you must:
- Have access to your own MyGeisinger account.
- Obtain the patient’s written approval (via form) or provide power of attorney documentation.
- Complete and return the appropriate authorization form.
Again, if you don’t have an account with us, one will be created for you so that you can access these records through MyGeisinger. Depending on whether you are a power of attorney (POA), you will have to fill out a form to obtain their medical records.
- If you are a POA, send/mail POA documentation with completed power of attorney form.
- If you are not a POA, complete the caregiver form.
Please note that POA documentation must be provided and put into the patient’s medical record for access.
