Pharmacy education

Inpatient pharmacists at Geisinger are educators as well as integral and active members of interprofessional care teams. While overseeing optimal, safe and cost effective medication therapy management using evidenced based medicine to improve patient care outcomes, they also actively participate in experiential education and training for pharmacy students, residents and fellows. Our pharmacists:

  • Provide accredited educational programming for medical, nursing and pharmacy professionals and students on a vast array of pharmacotherapeutic topics
  • Share didactic education at the bedside for our interprofessional care teams
  • Contribute to internal communications (newsletters and monographs) that promote the safe and effective use of medications
  • Offer patient and provider education

Direct patient care

At Geisinger, pharmacists and students participate in direct patient care. Nearly every inpatient unit and teaching service has a pharmacist dedicated to performing direct patient care activities each day. Disciplines with a clinical inpatient pharmacist dedicated to their rounding team include:

  • Medication reconciliation
  • Therapeutic plan development including medication selection, dosing, and monitoring for desired therapeutic as well as adverse outcomes
  • Therapeutic drug monitoring including pharmacokinetics consultations
  • Anticoagulation management and consultations
  • Inpatient pain management consultations
  • Adult code team participation (24/7)
  • Stroke team participation (24/7)
