The report - the first of its kind in the nation - identifies the actual numbers of infections reported by Pennsylvania's 168 hospitals, as well as other related quality of care measures. |

The Pennsylvania Health Care Cost Containment Council (PHC4) has released a report describing 30 conditions which specify the clinical reason(s) for a patient’s hospitalization. Of the 30 ICD9 code-based conditions, there are 19 diagnoses and 11 procedures. |
