At Geisinger, we care for the whole person. This includes your physical needs — and your behavioral and social needs.

That’s why we’ve included questions about social needs in our pre-visit screening process. The screening asks about urgent needs for food, housing, transportation, social connections, utilities, clothing, safety and more. The goal is to provide you and your family with the best care possible, and to connect you to resources that can help.

What are social needs? 

Social needs are conditions in the places where people live, learn, work and play that affect health and quality of life. Learn more here.

Why is it important for my care team to know about my social needs? 

Factors like finances, where you live and the things you do are part of your whole health. Talking about social needs lets us help you in all areas of your life.

How is Geisinger collecting information about social needs?

We recommend you fill out the social needs screening before your appointment in our patient portal, MyChart. You can also fill it out after you arrive at the clinic for your visit. The screening — which is optional — asks about your need for food, clothing, housing, social connections, utilities, safety, transportation, medication and childcare/caregiver support.
Your needs might change over time, so we ask the same questions every 90 days.  

What happens to my answers?  

The information will only be seen by Geisinger staff whose goal is keep you healthy. If you need some help and agree to receive it, our team will help you find local resources. 

What if I want to search for help on my own? 

If you’d like to find resources on your own, NeighborlyPA.com is a free website that allows you to search for free and reduced-cost programs and services close to home.
