LIFE Geisinger helps adults keep their independence

Eligible patients can get help with daily activities, medication reminders and care touchpoints. Transportation, meals, exercise and enrichment activities are also available.

Am I eligible?

To be eligible for LIFE Geisinger, a person must:

  • Be age 55 or older
  • Be determined by the local Area Agency on Aging to need the level of care required under the State Medicaid plan for coverage of nursing facility services
  • Live in our service area
  • Meet any additional program specific eligibility conditions imposed under the PACE program agreement 

LIFE is regulated by the Centers for Medicare and Medicaid Services and the PA Department of Human Services. LIFE Geisinger accepts Medicare, Medicaid and private payment. If you’re eligible for PA Medicaid, there is no premium. If you don’t have PA Medicaid, contact us for more information.

How do I enroll?

Our enrollment team makes the process simple, transparent and supportive — so you can feel confident in your decision to join.

Here’s what to expect:

  • Initial meeting: Meet with our enrollment team to learn about LIFE Geisinger’s services and benefits. If you choose to proceed, we’ll begin with a detailed intake assessment to understand your needs and preferences.
  • Clinical eligibility: We’ll guide you through the clinical eligibility process step by step.
  • Financial guidance: Our team will help you navigate the financial aspects of enrollment with clarity and care.
  • Meet the interdisciplinary care team: You’ll be introduced to our interdisciplinary team through center and home visits, guaranteeing a smooth transition into our program.

Disenrollment

Participants may initiate disenrollment from our services at any time. A disenrollment is effective on the first day of the month following the date we receive the participant’s notice of voluntary disenrollment.

Designating an appointed representative

An appointed representative is someone appointed by you, or authorized by the state or other applicable law, to act on your behalf in the grievance or appeals process in connection with your claim or asserted right under Title XVIII of the Social Security Act ("the Act") and related provisions of Title XI of the Act. The person you name to be your appointed representative can be a relative, friend, advocate or doctor.
