LIFE Geisinger offers services such as primary medical care and oversight, medication management and transportation to medical appointments.
And by getting to know you, our interdisciplinary team can create a care plan that meets your specific needs — while promoting independence, health and well-being.

You must be 55+, live in our service area and meet care and other requirements.
LIFE Geisinger's services
Our participants receive the support they need to be healthy, active and in their homes, from healthcare to medical transportation to light housekeeping.
What health and medical services are provided?
LIFE Geisinger provides services beyond medical care. Our interdisciplinary team will evaluate your needs and collaborate with you to develop a customized plan of care tailored to your health goals and lifestyle.
Services we provide include:
- Primary medical care and oversight
- Medication management
- Pharmacist oversight and counseling
- Transportation to our center and specialty appointments
- Home care, including light housekeeping, laundry and grocery shopping
- Nutrition counseling
- Help with food insecurity
- Social work intervention and support
- Physical, occupational and speech therapies
- Eye, hearing and dental care
- Respite care
- Medical advice and support during regular business hours
- Medical on-call services during evenings, overnight and on weekends
What center activities are offered?
Social engagement and physical activity are essential to well-being. Our centers offer programs that encourage movement, enjoyment and meaningful connections.
Activities at LIFE Geisinger include:
- Community outings to farmer’s markets, baseball games and amusement parks
- Entertainment by local musicians and artists
- Exercise and stretching
- Pet therapy
- Spiritual groups
- Gardening
- Self-directed activities
What does LIFE Geisinger cost?
If you’re eligible for Medicaid, there are no monthly premiums, deductibles or copayments for any of the approved care you receive.
If you’re eligible for Medicare but not eligible for Medicaid benefits, there is a monthly premium.
If you don’t have Medicare or Medicaid, there is an option to pay privately for our services.
All services authorized by your interdisciplinary team are covered in full for LIFE participants. LIFE participants must receive all needed healthcare, including primary care and specialist physician services (other than emergency services) from LIFE Geisinger or from an entity authorized by LIFE. LIFE Geisinger participants may be fully and personally liable for the cost of unauthorized or out-of-network services. Emergency services are covered and no prior authorization for these services is required.
