What is MyChart Medical Record Access?

MyChart Medical Record Access is an Internet application that enables a patient to have secure web-based access to portions of their electronic medical record, and allows a secure, convenient communications channel with participating physicians for non-urgent messages. use of this service is optional. Your login name and password are key to protecting the confidentiality of your information. It is your responsibility to keep your password confidential. It is only known by you and can be changed online at any time by you. Your password is not known by anyone at MyChart or your doctor's office.

Secure Messaging Guidelines

Please observe the following guidelines for messages sent through MyCareCompass:

Please DO NOT SHARE your password with anyone

Please do not send any messages requiring urgent attention. Typical turnaround time for responses is one (1) to two (2) business days.

Please note that messages may be viewed by other health professionals and others who work with your physician.


Your Responsibilities

By accessing or using MyChart you agree to be bound by all the terms and conditions of this Agreement and the MyChart Privacy Statement as posted. We may modify this agreement and the privacy statement at any time, and you agree that such modifications are effective immediately upon posting of the modified version.

You agree that MyChart is a communication service offered as a convenience to patients. You understand that it provides you online access to portions of your medical record and that you are solely responsible for any sharing of its content that you intentionally or unintentionally communicate to others. You understand that messages will be routed to the appropriate department as necessary for handling, and therefore employees, other than your healthcare provider may be involved in addressing your request. Therefore use of MyChart may not be appropriate for very sensitive information you wish to discuss directly with your provider.

You understand that you may receive messages, test results and other communication delivered via MyChart. Therefore, you may not receive other types of communications from your physician in these areas.


Discontinuing Use of MyChart Medical Record Access

You can dis enroll from your use of MyChart Medical Record Access at anytime by calling your physician office. If we determine at our sole discretion that you have violated this agreement and/or abused the use of this service, we reserve the right to discontinue your use of this service. You will be notified if we discontinue your MyChart Medical Record Access service.


Governance

This Agreement shall be governed by and construed in accordance with the laws of the Commonwealth of Pennsylvania.
