I need help accessing another person’s medical records 

Caregiver access allows you to view an adult patient’s medical records and communicate on their behalf. To request caregiver access, you must:
  • Obtain the patient's written approval to access their medical record or provide Power of Attorney documentation
  • Have your own patient portal account
  • Complete the appropriate caregiver authorization form:
Once you have completed your form, sign it and mail it to the address listed. We will notify you when your account has been set up, which usually takes 5 to 7 business days.
