14 –17 years old
As a parent of a minor (under age 18), you can view your child's medical record and communicate on their behalf. To request access, you must:
- Be the birth parent or legal guardian of a child under age 18
- Have your own MyChart account
- Complete and return the authorization form

18 years old or older
Caregiver access allows you to view an adult patient's medical record and communicate on their behalf. To request access, you must:
- Obtain the patient's written approval to access their medical record or provide Power of Attorney documentation
- Have your own MyChart account
- Complete one of the following authorization forms:
