Younger than 14 years old

As a parent of a minor patient (less than 18 years old), you can view your child's medical record and communicate on their behalf. To request access, you must:
Since your child is under age 14, you have two options:
  • Already have a myGeisinger account? Simply sign in to your account and click the Request Family Record Access link under the Profile tab. After you fill out your form, we will notify you when your access has been set up. This usually takes 5 to 7 business days.
  • You can also print and mail your form back to us or fax it to the number listed on the form. 

14 –17 years old

As a parent of a minor (under age 18), you can view your child's medical record and communicate on their behalf. To request access, you must:

  • Be the birth parent or legal guardian of a child under age 18 
  • Have your own myGeisinger account 
  • Complete and return the authorization form

Requesting access is easy. Simply sign in to your account and click the Request Family Record Access link under the Profile tab. After you fill out your form, we will notify you when your access has been set up. This usually takes 5 to 7 business days.
