My child turned 13 — I lost access or want to access their medical record

As the parent or legal guardian of a minor (under 18), you can view your child’s medical record, schedule and manage appointments, renew prescriptions and communicate with their care team. Children ages 13 to 17 can access their records with your approval.

Just follow these simple steps.
  1. Sign in
  2. On the menu, under “Sharing”, choose “Family Record Access”.
  3. Fill out the appropriate form based on your loved one’s age.
  4. Follow instructions to submit.

You’ll receive a response within 5 to 7 business days.

*Submit separate request forms for each child.
** If more than one parent or legal guardian would like access to their child's online medical record, each must complete a form.
