Discharge

When it is time for you to go home or perhaps to another facility, your doctor will write a discharge order.

A member of your Geisinger care team will go over instructions about how to care for yourself once you get home. You may want to have a family member or caregiver present so they too will know what to expect.

And remember, don’t ever hesitate to ask questions.

Social service representatives and case managers are also available to help patients and families cope with changes in their lives brought on by illness or injury and to discuss any concerns you may have about leaving the hospital. They can also help you find services in your community that may be able to provide additional help, if needed. If you or your family would like to talk with a social worker or case manager, ask a member of your care team about contacting a member of the Care Management Department.
