Heading home from the hospital
Discharge process
When you're ready to go home or to another facility, your doctor will write a discharge order.
A member of your Geisinger care team will give you instructions on how to care for yourself once you're home. They can also answer any questions you have. You may want to consider having a family member or caregiver present to help you digest information and ask questions, too.
Social workers and case managers will work with you on coping with illness and injury and will discuss any concerns you have about leaving the hospital. They can also help you find services in your community. Ask a member of your care team if you'd like to speak to a social worker or case manager.
Prescriptions delivered right to your room
There’s no need to stop at the pharmacy on the way home. Meds to Beds fills your prescriptions at select Geisinger locations and delivers them right to your room.
Your care team includes our pharmacy staff, who work closely with your doctors to:
- Review your medications, lowering the risk of unwanted interactions or duplications
- Make sure you’re getting the right medications at the right times
- Help resolve insurance issues
- Get you started on medications your local pharmacy might not have in stock

We accept most major insurance plans. Copays are due at the time of delivery. You can pay by cash, check, credit or debit card or flex spending card. If you don’t have a form of payment or the means to pay, don’t worry. We’ll work with you to find options.
