With same-day appointments and our 24/7 phone line for non-emergency health questions, you’ll get the care you need when you need it.

Labs, imaging and nutrition advice are all under 1 roof, so you can have a checkup, a blood draw and a dietitian visit in a single trip. Our centers go beyond clinical care, too. Geisinger 65 Forward is available to adults 55 and older enrolled in Geisinger Commercial plans, Geisinger Gold plans and Dual Eligible Special Needs Plans (D-SNP).

Is Geisinger 65 Forward only available to Geisinger Health Plan members?

Yes. Geisinger 65 Forward is only available for adults 55 and older enrolled in Geisinger Commercial, Geisinger Gold and D-SNP plans.

Are exercise classes (and other offerings) open to non-members?

No, our services are for members only. Geisinger 65 Forward patients have exclusive access to wellness offerings — including exercise classes, fitness equipment and wellness programs — plus clinical services like lab draws and X-rays. 

Do I have to change my primary care provider to join Geisinger 65 Forward?

Yes. When you join 65 Forward, you’ll see one of the primary care providers (PCPs) at the center you choose. Our centers are led by teams who specialize in healthcare for older adults, and every healthcare professional plays a role in your care — from helping you improve your fitness to lowering your cholesterol. Your team will include a:

  • Doctor or other PCP who gets to know you and oversees your overall health
  • Nurse who works alongside your doctor and tailors your care to your medical needs
  • Nutritionist who reviews your diet and offers tailored meal plans
  • Wellness coordinator who creates a custom fitness plan
  • Pharmacist who reviews your medications to make sure they work well together and supports treatment for ongoing health conditions
  • Community health worker who connects you with community resources, reducing barriers to care
  • Healthcare advocate who coordinates your care and helps you maximize your Medicare benefits

How do you become a Geisinger 65 Forward patient?
