Help close diabetic eye exam care gaps for your patients

We’re currently reaching out to members to encourage completion of their annual diabetic eye exams. This outreach is part of our ongoing effort to close care gaps and improve health outcomes for our diabetic population.

We understand that some members may have already completed their diabetic eye exams with an ophthalmologist or optometrist. However, in many cases, we do not have the necessary documentation on file to reflect this in our records.

How you can help

For accurate reporting and to support your patients’ care:

• Verify that your patients’ medical records include documentation of their most recent diabetic eye exam and location.
• If the exam was completed outside your practice, make sure results from the ophthalmologist or optometrist are incorporated into the patient’s chart.

Why this matters

Closing the diabetic eye exam care gap not only improves patient care but may also positively impact your quality incentive payment.
