Gloria Gates Care location in Berwick to close Sept. 11

An upcoming facility closure will affect Geisinger Health Plan members receiving services at the Gloria Gates Care location in Berwick.

Effective Friday, Sept. 11, Gloria Gates Care at 751 E. 16th St. in Berwick will close and will no longer be available to Geisinger Health Plan members.

Member outreach and continuity of care

We’re notifying affected Geisinger Health Plan members of the closure and encouraging them to select a new participating provider soon to avoid care interruptions. Anyone needing help finding a new provider can call Geisinger Health Plan Customer Care at 800-447-4000 or visit geisinger.org/health-plan/find.

Members can contact Customer Care after selecting a new provider to confirm the provider participates in the Geisinger Health Plan network.
