My child turned 14 — I lost access to their medical record or would like access to their medical record
As a parent of a minor patient (under age 18), you can view your child's medical record and communicate on their behalf. To request access, you must:
- Be the birth parent or legal guardian of a child under age 18
- Have access to your own MyChart account
- Complete and return the appropriate parental authorization form
If you already have a MyChart account, simply sign in to your account and click the Request Family Record Access link under the Profile tab. After you fill out your form, we will notify you when your access has been set up. This usually takes 5 to 7 business days.
