Geisinger Triple Choice Formulary

Prescriptions are assigned to 1 of 3 tiers, each with a set copay. Additional medications may be covered under the Triple Choice benefit depending on your prescription rider.

Geisinger Traditional Formulary

Prescriptions have either a flat copay/coinsurance, 1 copay for generic or 1 copay for brand, or assigns them to 1 of 2 tiers. The copay amount is based on your prescription rider. Other medications may be covered under the Traditional benefit.

Geisinger High Performance Rx Formulary

Prescriptions are assigned to 1 of 3 tiers, each with a set copay based on your medication rider. Additional medications may also be covered under the High Performance Rx benefit.

Geisinger 4th Tier

Prescriptions are assigned to 1 of 4 tiers, each with a set copay based on your prescription rider. Additional medications may be covered under the 4th Tier benefit.

Aon Benefit Experience

Prescriptions are assigned to 1 of 3 tiers, each with a set copay dependent on the level and pharmacy. Details are defined in your employer's benefit materials.
