GHP Kids Member Rewards Program

Earn a variety of rewards just for keeping your child up to date on well visits, routine vaccines, dental visits and health screenings. It's easy.

Once your child completes these health activities, you'll earn a gift card. You don't need to do anything else.

Well visit appointments:

Enrollees are eligible for a:

$15 Walmart Healthy Living gift card*

• For going to 6 well visit appointments before age 15 months.
• Receive another $15 gift card if they go to 2 more visits between 15 and 30 months.

$30 Walmart Healthy Living gift card*

• For going to 1 well visit each year from age 3 to 17.

Immunizations for adolescents:

$25 Walmart Healthy Living gift card.*

• Enrollees are eligible once all adolescent vaccinations are completed:
  • 1 meningococcal vaccine (between ages 11–13)
  • 1 Tdap vaccine (between ages 10–13)

$30 Walmart Healthy Living gift card.*

• 2 HPV vaccines (between ages 9–13)

Annual dental visit:

$25 Walmart Healthy Living gift card.*

• Complete 1 dental checkup with a licensed dental provider during the calendar year.

Lead screening for infants and toddlers:

$10 Walmart Healthy Living gift card.*

• Enrollees age 9 months or older are eligible if they complete 1 capillary or venous blood test for lead poisoning before their 2nd birthday

*Gift cards may not be redeemed for cash value, regardless of dollar amount, and cannot be used to purchase alcohol, tobacco, firearms or ammunition, optical, prescriptions, candy, items at register aisles, electronics, toys, seasonal items or jewelry.
