What is AMP?
Geisinger’s Anticipatory Management Program (AMP) is a weblink under Workflows for this Plan on NaviNet that supports clinical decision-making around chronic conditions and potentially untreated disease.
AMP provides insights into untreated chronic conditions and transparency into diagnoses by other providers. It also identifies relevant gaps in care that need to be addressed each year.
AMP packages all this useful patient information into a concise format that can be easily integrated into a provider’s workflow.

We’re hosting two virtual provider townhalls* for physicians and office staff to demonstrate the various ways AMP can bring value to your organization and help manage your Geisinger Health Plan patients. Choose the one that fits your schedule:

Already using AMP?
We’re adding a selective list of care gap measures to your AMP reports from NaviNet.
The care gaps section will be located at the bottom of a patient’s AMP report:
• All care gap measures applicable to that patient will be displayed.
• When no care gaps information is available for a patient, “no available gaps” will be displayed.
Care gaps will display a check box with:
• Open care gaps will have an unchecked box
• Closed care gaps will have a check mark
Here is a list of possible care gaps for AMP reports through NaviNet you can expect to see:
• Statin therapy for patients with cardiovascular disease (SPC)/ received statin therapy
• Breast cancer screening
• Colorectal cancer screening
• Eye exams for patients with diabetes
• Blood pressure control for patients with diabetes
• Hemoglobin a1c control for patients with diabetes
