What is Geisinger 65 Forward? Senior primary care (and so much more).
Geisinger 65 Forward is designed exclusively for people 65 and older. Our health and wellness centers offer everything you need under one roof in a calm, relaxing environment that feels more like a spa than a clinic. You’ll get:
Same-day appointments: See your doctor faster and get the attention you deserve.
Longer visits: More 1:1 time with your doctor (up to 1 hour).
Everything under one roof: On-site lab, imaging, mental health services and nutrition guidance — our centers are a one-stop shop for all your health needs.
Social + educational activities: Yoga, ceramics, card games and other activities fill your calendar and keep you healthier and happier — for free, and in the same location as your doctor.
A personal wellness plan: State-of-the-art fitness equipment, exercise classes and wellness coordinators help you stay on-track with your goals — all integrated with your Geisinger Gold health plan.

All Geisinger 65 Forward services — primary care, lab, radiology, wellness classes, nutrition help, rotating specialties, arts and crafts and more — are covered for our Geisinger Gold (Medicare Advantage) members. Because Geisinger offers its own insurance plan, we can seamlessly deliver this high-quality care to our members.

The fitness equipment and events we offer at 65 Forward are only for our patients.
If you’re a new patient and have an appointment scheduled, you’re welcome to use the fitness center or attend a wellness class before your appointment.
Once you’re enrolled in 65 Forward, we encourage you to take part in fitness, wellness and social activities. Studies show that many people 65 and older become socially isolated, which can affect their physical and mental health. That’s why we encourage our patients to gather at the center for activities like card games, painting sessions and book clubs.
Lab draws, X-rays and other medical services are also reserved for 65 Forward patients.
Yes. When you join 65 Forward, you’ll need to see one of the providers at the center you choose.
The good news? You’ll be gaining a Geisinger doctor who specializes in caring for people 65 and older — and a whole care team that’s focused on your health and wellness.
Your team will include:
- A nurse who works alongside your doctor and knows you and your medical history.
- A care coordinator/coach who’ll make sure you get the right care at the right time, based on your unique medical needs.
- A social worker who will work with you and your family to coordinate non-clinical needs like counseling.
- A nutritionist who will review your diet and recommend healthy choices.
- A wellness coordinator who partners with your doctor to create a fitness plan tailored to you.
- A healthcare advocate who works with you, your family — and your specialists, hospitals and Medicare plan — to coordinate your care and help you take advantage of the health benefits and services we offer.
We offer a new patient orientation where you can learn about the program and meet our providers. To qualify, you must be 65 or older and have Geisinger Gold (Medicare Advantage) insurance. You don’t need a referral, but you will need to switch from your current primary care provider (PCP) to one of our 65 Forward providers.

No. Geisinger 65 Forward has no out-of-pocket expenses.
To participate, you need to be 65 or older and have Geisinger Gold (Medicare Advantage) insurance. We also accept Tricare (insurance for military veterans).
All the services provided by Geisinger 65 Forward are covered by insurance with no additional costs.
