Supporting safe discharge planning for GHP Family members
Hospitals and discharge planning teams play a critical role in ensuring safe, timely, and well-coordinated discharges for GHP Family members — including transportation needs.
We work with hospitals to develop discharge plans that prioritize member safety and continuity of care, including reliable transportation arrangements.
Key expectations for hospitals
Start transportation planning early
Assess transportation needs at the time of admission and discuss them with the member throughout the inpatient stay. Early planning helps prevent delays and supports a smooth transition home or to the next level of care.
Verify MATP enrollment
Confirm whether the member is enrolled in the Medical Assistance Transportation Program (MATP), which provides non-emergency transportation for those who are eligible. Use the MATP website or PennDOT’s Find My Ride site to verify enrollment.
Promote early enrollment when needed
If the member isn’t enrolled in MATP, encourage enrollment as early as possible. Our Enhanced Member Supports Unit (EMSU) can help members enroll and navigate available transportation resources.
Document transportation arrangements
For members who don’t have reliable transportation, include a safe and clearly documented transportation plan in their discharge plan. This ensures accountability and continuity of care post-discharge.
Ensure discharge readiness
Don’t discharge members without confirmed, safe transportation. When transportation barriers exist, extra coordination or resources may be necessary. In some cases, an extended length of stay may be appropriate to ensure a safe discharge.
