Join the Fresh Food Farmacy program

This mobile app was designed specifically to support patients of the Fresh Food Farmacy program on their journey to better health.
Interested in the program? Here’s how to join.

There are a few simple requirements:

  • You’re at least 18 years old.
  • You’ve been diagnosed with Type 2 diabetes.
  • Your A1C is higher than 8.0.
  • You regularly see a primary care provider.
  • Paying for the food you need is a challenge.
  • If enrolling at a satellite location, participant must see have Geisinger Health Plan insurance.

If Fresh Food Farmacy sounds like a good fit for you, talk to your primary care doctor.

If you are an existing patient and are having trouble logging in to the Fresh Food Farmacy app, talk to your care team.
