Home care program receives $39,000 from the Ralph and Josephine Smith Fund

Danville, PA – Geisinger at Home has been awarded a $39,000 grant from the Ralph and Josephine Smith Fund at the First Community Foundation Partnership of Pennsylvania to continue providing personalized healthcare to eligible Geisinger Gold Medicare Advantage members right in their homes.

Geisinger Health Plan’s at-home care program helps keep patients healthier, safer and better connected to their healthcare team. Working closely with patients’ primary care physicians, Geisinger’s team of doctors, registered nurses, dietitians, case managers, pharmacists and other medical support staff can visit patients as needed, right in their very own home.

These regular home visits ensure patients are meeting their health needs before becoming an issue that requires a trip to the emergency room or hospital.

“The First Community Foundation Partnership of Pennsylvania has a long history of creating positive impact within our communities, and we are grateful for their partnership and support,” said Nancy Lawton-Kluck, Geisinger Health Foundation’s chief philanthropy officer. “This latest gift from the Ralph and Josephine Smith Fund will help many patients in the Geisinger at Home program, which is designed for individuals who have complex and difficult-to-manage health conditions. Our program brings a wide range of services and exceptional providers right to a patient’s home.”

More than 4,000 GHP members in 15 counties are currently enrolled in Geisinger at Home, and the program has reduced emergency room visits by 43 percent and hospital admissions by 48 percent, per 1,000 patients.

Grants and scholarships from the Ralph and Josephine Smith Fund are made possible by the generous gifts of Josephine Smith, a woman of vision for the future of her community. Charitable dollars are available yearly to four designated organizations that serve Northumberland County.
